Essay

The dissertation is dedicated to the problems of surgical treatment of critical leg ischemia due to occlusive-stenotic lesions of tibial arteries, and to improvement of the surgical strategy. The study was based on comprehensive clinical, instrumental and laboratory assessment and analysis of immediate and long term outcomes in 301 patients with critical leg ischemia secondary to occlusive-stenotic lesions of tibial arteries. Control group (group I) included the patients who underwent the standard and generally accepted surgical procedures. The study group (group II) included the patients, who underwent surgical treatment based on the improved techniques. The patients were followed for 10 years, mean follow-up 5.3±2.7 years. The patients were predominantly male, the age was in the range of 20 to 85 years, the mean age was 63.10±0.68 years. Occlusive-stenotic lesions of leg arteries were caused predominantly by atherosclerosis and much less frequently by post-embolic occlusions, atherosclerotic aneurysms and endarteritis.
