The dissertation is devoted to the problem of optimization of surgical treatment of age-related cataract based on the investigation of the features and mechanisms of the development of postoperative macular edema and its connection with the system of intravascular fluid prostanoids, as well as the development of a new method for its prediction. Significant criteria for forecasting the development of macular edema after surgical treatment of age related cataract are developed - age, CDE index, time of intraocular phase of operation, ratio of ratio of 6-keto-PGF1alfa / thromboxane. The criteria of risk of macular edema development are revealed, allow to make an assumption about the development of this complication in the pre - and intraoperative stage, which in turn make possible to prepare and notify the patient about the possibility of complications and low visual acuity in the postoperative period. The methods of predicting the development of сentral retinal edema, as well as the causes that can cause them, are important for optimizing the management of patients in the postoperative period and allow you to make an individual prognosis of complications for each patient and make the necessary measures aimed at early diagnosis, timely prevention and treatment.
