In the dissertation theoretical generalization and a new solution of the modern issue of internal diseases are presented: optimization of diagnosis and treatment of gastroesophageal reflux disease (GERD) with hypertension (GC) in the liquidators of the ChNPP accident by studying the features of clinical course, apelin-12 levels, melatonin, interleukins ( 1-beta, -6), indicators of quality of life, structural and functional state of the myocardium with additional inclusion in the complex therapy of Actovegin and developed a model for assessing the effectiveness of treatment. Supplemented data on the accumulation of clinical course when combined GC from GERD to the liquidators of the Chernobyl accident. More frequent complaints of heart palpitations and cardiac arrhythmias were detected, higher DAT figures were established, violations of myocardial repolarization processes were recorded according to electrocardiography data. The main limitations of quality of life on the scale of pain and the indicator of the general state of health, with the combination of the GC of the GERD, with the liquidators of the Chernobyl accident, have been proved. The data of the influence of the combined pathology on the structural and functional parameters of the myocardium (Gpmax E, Vmax A, DLP, DAC LV, DPSH, KDO LV, KSO LV, V max E / V max, G max max, FV) have been refined from the liquidators of the accident on ChNPP, patients with GC with concomitant GERD. It has been proved that the combination of GC with GERD leads to increased concentration of apelin-12 levels, decreased secretion of melatonin, increased activity of inflammatory markers (interleukin-1-beta, -6) in Chernobyl accident liquidators. New data were obtained that allowed to optimize the therapy of GC with concomitant GERD and proved the expediency of using the actovegin drug in the complex therapy at the Chernobyl disaster liquidator. The model for assessing the treatment effectiveness of the Chernobyl Nuclear Accident Liquidators, patients with GC with concomitant GERD with the characteristics of the clinical course, indicators of the structural and functional state of the myocardium, quality of life, apelin-12 levels, melatonin, interleukins -1-beta, -6 .
