Hrynkiv N. Optimization of complex treatment of patients with acute necrotizing pancreatitis using extracorporal methods of detoxification and immunocorrection.

In the dissertation the results of complex treatment of patients with acute necrotizing pancreatitis is represented. There were improved diagnostic and treatment tactics, determination of factors that negatively influence the course of the disease. There were represented methods of preventing complications development based on the analysis of clinical symptoms, investigation of functional state of microcirculation and level of endogenous intoxication. The results of investigation of microcirculation, endogenous intoxication and the dynamics of cell and humoral immunity in patients with acute necrotizing pancreatitis, depending on the severity of the course and the phases of the disease, are represented. The main factors influencing the severity of the course of acute necrotizing pancreatitis and the development of complications are determined.The influence of extracorporal methods of detoxification and immunocorrection with inclusion in the treatment of water soluble form of quercetin on the dynamics of clinical parameters, microcirculatory disorders and markers of endogenous intoxication has been investigated. The use of the recomended therapy allowed to renew motor and evacuation function of the gastrointestinal tract faster, to reduce the terms of hospitalization and mortality rate in the main group of patients with acute necrotizing pancreatitis.
