Essay

The object of research: the secretory function of the pancreas, trophological status, endogenous intoxication, immune status, microbiocenosis of the large intestine and quality of life in 93 patients with chronic pancreatitis in comorbidity with ascariasis and in 32 patients with chronic pancreatitis. The purpose of the research: to improve the methods of differential diagnosis and prognosis of concomitant ascariasis in patients with chronic pancreatitis, as well as to increase the efficiency of complex treatment and rehabilitation by additional use of enterosorbent and synbiotic in the outpatient surveillance program, established on the basis of the analysis of clinical and pathogenetic characteristics of the course of this comorbid pathology. Methods of research: clinical-anamnestic; anthropometric - body mass index (BMI), shoulder muscle flexing (SMF), skin-fat fold over triceps (SFOT); laboratory - number of red blood cells, hemoglobin level; biochemical - blood glucose, total protein, blood amylase; coprogram; medium mass molecules (MMM) and erythrocytic index of intoxication (EII); microbiological examination of feces; immuno-enzymatic - fecal ?-elastase, CD3, CD4, CD8, CD16, CD22, IgG, IgA, IgM, IgE; instrumental - ultrasound (ultrasound) software; sociological - questionnaires on international scales SF-36 and GSRS; statistical ones. The scientific novelty of the obtained results: the understanding of the pathogenetic mechanisms of the formation of trophic insufficiency, namely, the depletion of the visceral and somatic protein pools in patients with CP with accompanying ascariasis, is deepened, the role of disturbances of the intestinal microflora, imbalance in the immune status and EE in the course of the pathological process has been proved. For the first time, the dependence of the imbalance of the microflora of the colon on the depth of violations of the exocrine function of PO in the case of HP with accompanying ascariasis was performed. The violation of the relationship between immune status, ES, dysbiosis of the colon and the development of TN is shown. For the first time, the effectiveness of complex therapeutic and rehabilitation programs of medical correction of revealed disorders with the use of complex enterosorbent, bioanorm and synbiotic rotabiotoxic has been proposed and proved on the basis of improvement of clinical course of CP, secretory functions of PO, decrease of manifestations of TN and endotoxicosis, positive effect on colon microflora and immune status, and as well as the growth of the physical and mental components of QOL in the process of treatment and rehabilitation of patients with HP with concomitant ascaridosis.
