Essay

The dissertation is devoted to the optimization of management of young age patients with the grade 1 stage I arterial hypertension with excess body weight in GP practice. The frequency of traditional and additional risk factors of cardiovascular diseases among this cohort of patients was analyzed. Significant influence of psychoemotional changes on the level of cardiovascular risk was determined. A number of additional factors that reflect the hormonal activity of adipose tissue and metabolic disorders (leptin, TNF-α, VCAM-1, HOMA) were determined, and found the effect of these factors on endothelial dysfunction, which confirms the theory of immune-inflammatory genesis of metabolic syndrome and hypertension. A nomogram model of the dependence of leptin level and body mass index has been developed, which allows a general practitioner to approximate the level of leptin among such patients. The effectiveness of a comprehensive treatment and prevention program for the correction of excess body weight and blood pressure among young patients with I stage hypertension was determined, which included dietary nutrition, physical activity and taking a complex naturopathic drug Homviotensin. Peculiarities of commitment to the implementation of the provided recommendations and the influence of the level of commitment on the dynamics of body weight and blood pressure was determined. It was established that the use of short motivational consultations by a family doctor in the process of managing such patients increases the level of commitment and effective achievement of blood pressure level normalization and reduction of body mass index.
