Havrysh R. Gastric plication. Clinical and metabolic aspects (clinical and experimental research)

The dissertation is dedicated to justification of gastric plication treatment results in patients with obesity and manifestations of metabolic syndrome. A comprehensive research was conducted to study the influence of laparoscopic gastric plication surgery on parameters of lipid and carbohydrate metabolism in obese patients, as a result of which new scientific data were obtained. The morphofunctional changes of pancreatic tissue after gastric plication surgery in rats with streptozotocin-induced diabetes mellitus was studied. It was proved that gastric plication induces regeneration of pancreatic islets В-cells and restores pancreatic В-cells insulin granules content in rats with streptozotocin-induced diabetes mellitus. For the first time was proposed perform upper GI endoscopy during laparoscopic gastric plication to determine the internal volume of the stomach as a criterion of restriction and to improve effectiveness of surgery.It was scientifically shown that patients underwent laparoscopic gastric plicaciation had decrease of body weight, normalization of lipid metabolism parameters, breathing improving and blood pressure normalization. It was scientifically proved that some patients with obesity and type 2 diabetes mellitus after gastric plication had long-term carbohydrate metabolism normalization (glucose, gycated hemoglobin parameters).
