Essay

The thesis is devoted to the substantiation of choosing the optimal complex of long-term antihypertensive therapy for pregnant women with arterial hypertension, which allows such patients to provide a reserve of compensatory mechanisms of vascular regulation, and end the pregnancy with the birth of healthy children. The study allowed to improve timely diagnosis of the degree of cardiovascular and obstetric risk in pregnant women with arterial hypertension. It was revealed that the immediate cause of the complicated course of pregnancy and childbirth in pregnant women with arterial hypertension is a violation of the uteroplacental hemodynamics due to endothelial dysfunction, which contributes to a significant imbalance in the functioning of the prooxidant-antioxidant systems. For the first time, it was proved that the inclusion in the comprehensive treatment program for pregnant women with arterial hypertension and high cardiovascular risk of highly selective b-blocker and L-arginine ensures the achievement of the target level of blood pressure and restoration of central hemodynamics and uteroplacental perfusion due to normalization of endothelial function and balance in the functioning of the antioxidant system protection, accompanied by a decrease in the frequency and severity of complications in the mother and fetus.
