Essay

The object of study: early markers of kidney damage in essential arterial hypertension, functional state of the endothelium. The aim of the study: to increase the effectiveness of prevention and diagnosis of kidney damage in patients with stage I essential hypertension, grades 1-3, by determining the functional renal reserve by the method of water-salt loading. Research methods: laboratory; instrumental; statistical. Scientific novelty of the results: for the first time, the main factors that increase the likelihood of reduction and loss of functional renal reserve in patients with stage I essential hypertension were identified. On the basis of taking into account the obtained clinical and laboratory indicators in the quantitative evaluation system, a method for early diagnosis of renal function decrease in patients with stage I essential hypertension was proposed; Algorithm-scheme of management of patients with essential arterial hypertension of the first stage with low and moderate risk of development of cardiovascular complications at the stage of primary medical care on the basis of their distribution on the severity of decrease in renal function is substantiated and proposed. It has been demonstrated for the first time that the early onset of drug antihypertensive treatment has a positive effect on kidney function in patients with stage I low and moderate risk of cardiovascular complications.
