Ovcharenko O. Optimization of the results of surgical interventions on the breast taking in to account prognosis and correction of postoperative lymphorrhea.

The dissertation contains theoretical basis and offers a practical solution to a scientific problem - improving the results of complex surgical treatment of patients after different kinds of mastectomia with regional lymphodissection by the way of prognosis of risk and prevention of development of postoperative lymphorrhea. For practical application, a method of prognosis of the duration and volume of lymphorrhea, depending on immunological and biochemical changes was proposed. The applied zigzag-access technique to mobilize the breast during radical mastectomy provides good tissue adaptation and elimination of "free" cavities. The original technology of high-frequency electrosurgical welding during breast mobilization and lymphodissection is developed and implemented. The processing of the operating field by contactless thermosetting of living tissues and the use of a biodegradative collagenous hemostatic plate in the complex of prevention of lymphorrhea and the elimination of "free" cavities is improved. The technique of glucocorticoid therapy and compression effects in the postoperative period for the patients with the prognosis of the long and high volum post-macectomy lymphorrea was first introduced.
