Essay

The dissertation is devoted to the decision of the actual issue of improvement of the results of surgical treatment of inguinal hernias by improving the technique of transadominal preperitoneal alloplasty (TAPP). It consists of clinical and experimental sections. In the experimental section, the possibility of fixing the mesh with glue based on cyanocrylate was explored. The results show that the use of adhesive is effective. The clinical section included 240 patients, which were divided into three groups: Group I - classical TAРР technique with mechanical mesh fixation was performed; Group II - we have developed a method of TAРР with combined fixation; Group III - our advanced TAРР technique with adhesive fixation. The use of TAРР with combined mesh fixation reduces the incidence of relapses from 7.6 % to 1.6 %, chronic postoperative pain from 7.5 % to 3.1 %, and improved TAРP with adhesive mesh fixation reduces recurrence of inguinal hernia from 7.57 % to 1.5 %, and excludes the occurrence of chronic postoperative pain.
