Essay

The dissertation is devoted to the optimization of treatment for patients with psoriasis (P) and activated Herpes simplex virus infection, types 1,2 (HSV1,2) by the development of advanced tactics for complex differentiated antiviral therapy, taking into account molecular, genetic, and immunological mechanisms of these diseases. It has been found the clinical features of the disease in patients with P+HSV 1,2 compared with the patients with only P, especially according to the PASI index (р<0.05). It has been determined the molecular, genetic, and immunological features of the changes, characterized by lower expression of miR146a (p<0.05), lower number of regulatory lymphocytes (p<0.05), lower level of TGF? (p<0.05), and features of IL23 and IFNa synthesis in saliva and blood. It has been proposed the model of prediction on risks of chronic HSV1,2 activation in patients with P, the number of HSV1,2 recurrences during the year, and the features of psoriasis with HSV1,2 by the nature of its exacerbation frequency. It has been developed an advanced method of treatment for patients with psoriasis and HSV1,2, providing the use of inosine pranobex and/or acyclovir on the background of standard therapy, depending on the HSV1,2 severity, that contributes to the improvement of clinical and immunological efficacy and safety of dermatosis treatment.
