Essay

This thesis represents theoretical generalization of the results and achieves scientific objective – to prove the role of vascular endothelial dysfunctions (vascular endothelial growth factor, endothelin-1, thromboxane-A2, prostacyclin parameters) and rheological properties of blood (superficial viscosity, elasticity, viscoelasticity parameters) in pathogenetic development of combustion process; to determine optimal individual patient-management strategies (tumescent administration of epinephrine and ozone-oxygen mixture during surgical and ultrasound necrosectomy) according to performed experimental morphological studies of angiopathy in animals with burn injury (60 rats, 44 rabbits) and clinical laboratory developments in 117 patients suffering from grade ІІб-ІІІ thermal injury. Informative criteria were specified, providing an opportunity to predict burn injury progression and results of therapeutic measures, combined ultrasound necrosectomy with the use of ozone-oxygen mixture demonstrated the highest effectiveness. Key words: burn injury, experiment, vessels, endothelium, blood, rheology, treatment, necrosectomy.
