Essay

The dissertation is devoted to improving the diagnosis of non-alcoholic fatty liver disease by establishing clinical, biochemical and morphological features of the liver, and based on the study of the relationship between various variants of structural changes in the liver, certain invasive and non-invasive methods. In the study of 122 patients with NAFLD including 15 patients with liver (LS) steatosis, 92 patients with steatohepatitis (SH) and 15 patients with cirrhosis (LC), an increase in the level of hyaluronic acid, α2-macroglobulin and fibronectin (r = 0.72, 0.93 and 0.71 at p <0.05 respectively) and decrease in the level of apolipoprotein A1, haptoglobin (r = -0.61 and -0.35 at p <0.05) depending on the stage of fibrosis in patients with SH and LC. A direct relation was established between the degree of activity of the inflammatory process in patients with SH and LC, for hyaluronic acid, α2-macroglobulin and fibronectin (r = 0.54, 0.67 and 0.55 for p <0.05), and reverse linkage For patients with SH the diagnostic value of α2-macroglobulin, apo-lipoprotein A1, hyaluronic acid and fibronectin for which the area under The ROC curve for minimal fibrosis was not less than 0.75, and for moderate, severe fibrosis was greater than 0.9. Patients with LC have a high sensitivity level (96.4 -100.0%) and specificity (75.0 - 100.0%) of these markers.
