Essay

The thesis is devoted to optimization of diagnostics and predicting of treatment effectiveness in patients with stage II essential hypertension based on the investigation of the influence of modifiable risk factors (RF) on hemodynamic parameters, cardiovascular remodeling and the state of autonomic balance. It was established that the presence of individual modifiable RF and their combinations in patients with stage II hypertension leads to the formation of specific changes in office blood pressure (BP) of the daily profile of BP, structural and functional restructuring of the heart and blood vessels, violation of heart rate variability. The effect of telmisartan and eprosartan on systemic, cerebral, intracardiac hemodynamics, structural and functional remodeling of the heart and blood vessels, on autonomic balance in patients with hypertension, taking into account individual RFs, has been clarified. The dependent and independent predictors of antihypertensive therapy ineffectiveness in patients with hypertension were identified.
