The thesis focuses on the improvement of HT treatment effectiveness in the setting of COPD on the ground of structural-functional characteristics of PA and RV study, pathogenetic mechanisms of their disorders and effectiveness of pharmacotherapy with amlodipine in a combination with rosuvastatin on the background of a baseline therapy. For the first time structural-functional values of stiffness of PA and right compartments of heart, vegetative dysfunction, level of high sensitive C-reactive protein, matrix metalloproteinase-9, endothelin-1 were studied in patients with a comorbid course of HT and COPD. The study also revealed peculiarities of the disorder of PA elastic properties in patients with a comorbid course of HT and COPD, disorders of myocard deformative features of the right and left ventricles. Clinical effectiveness of rosuvastatin and amlodipine besylate addition to the baseline therapy of patients with HT and COPD in a course of 24 weeks was proved relating to the positive effect on the studied pathogenetic, morpho-functional and biochemical factors with more substantive values normalization of the pulmonary artery and right ventricle.
