The thesis explores the effect of obesity on the course of chronic obstructive pulmonary disease (COPD). COPD and obesity were found to have a mutually aggravating effect on each other, which is characterized by a higher comorbidity burden by Charlson comorbidity index associated with the systemic inflammatory response activation, more marked bronchial obstruction and more severe pulmonary insufficiency, and is accompanied by depression. It was discovered that in the event of hyperleptinemia, COPD course is unfavourable, which is manifested by a significant impairment of respiratory function with obstructive and restrictive disorders, inflammation syndrome activation, development of depression with anxiety features and a lower quality of life. The most pronounced bronchial obstruction is observed for cases of raised leptin and tumor necrosis factor alpha levels together. When COPD is combined with obesity, adaptation processes are disturbed, which is displayed by the prevalence of stress response and unsatisfactory adaptation associated with inflammatory response activation and impaired bronchial patency. The adaptation index measures the severity of inflammation and endogenous intoxication, allows assessing the immune reactivity, while its lowered value is associated with increased levels of tumor necrosis factor alpha. The use of African geranium root extract with the standart basical COPD therapy promotes the regression of bronchial obstruction, especially in obesity cases.
