Essay

The thesis deals with the practical substantiation and the new solution of the scientific problem of medical rehabilitation, physiotherapy and health resort, which is related to the increase of the efficiency of the complex rehabilitation (at the inpatient and outpatient stages) of the postmenopausal women suffering from osteoarthrosis of the knee by developing differentiated treatment and rehabilitation complexes taking into account clinical course, X-ray stage of osteoarthrosis, markers of inflammation and genetic markers. The work was performed on the basis of complex clinical laboratory evaluation and further treatment and rehabilitation of 220 postmenopausal women with osteoarthrosis of the knee, a comparison group — 100 postmenopausal women without osteoarthrosis of the knee who underwent inpatient treatment in the cardiology and rheumatology departments of the General Medical Center of the ONMedU (Odesa). For the first time it was proved that introduction of chondroprotective and anti-inflammatory drugs in combination with neocaripazim-400 electrophoresis and pressotherapy into the course of complex therapy at the short-term and long-term periods of treatment improved the prognosis and quality of life of the postmenopausal women with osteoarthrosis. The study specified and supplemented the data on the features of the clinical course of osteoarthrosis of the knee in the postmenopausal women and the influence of genetic factors, risk factors on the development and course of osteoarthrosis of the knee in the postmenopausal women. The data on the state of the articular cartilage, bone density, the presence of inflammatory fluid, joint mobility (passive and active), the severity of pain and inflammation, as well as the state of lipid and carbohydrate metabolism in the postmenopausal women with osteoarthrosis of the knee were elaborated and supplemented. The effect of complex therapy of chondroprotective and anti-inflammatory drugs in combination with neocaripazim-400 electrophoresis and pressotherapy in the postmenopausal women suffering from osteoarthrosis of the knee was studied for the first time. The practical importance of the thesis is that based on the conducted research, the clinical approach to the choice of therapy aimed at improving the metabolism of bone, adipose tissue, the state of glucose metabolism and restoration of a satisfactory quality of life of the postmenopausal women with osteoarthrosis of the knee is substantiated. The inclusion of the developed methods helps to reduce the medicamentous load on the patient.
