Essay

The thesis is focused on the type of central (CH) and peripheral (PH) hemodynamics, as well as on their correlation with vegetative regulation of the cardiovascular system in hypertensive patients with concomitant obliterating atherosclerosis of the lower extremity arteries. Hyperkinetic and eukinetic CH type alongside with sympathetic type of vegetative nervous system (VNS) have been found to prevail in most patients with this comorbid pathology. Reconstructive surgery has been found to result in the change of CH type, requiring due regard for the administration of antihypertensive therapy. Reconstructive surgery-induced change in the CH type has been proved dangerous in view of developing ischemic disorders in the lower extremities. Recommendations as to the choice of effective complex antihypertensive therapy by determining an efficient combination of basic antihypertensive remedies (in particular, carvedilolum - hydrochlorothiazide, lisinopril - amlodipine, telmisartan - hydrochlorothiazide) in the outpatient practice, based on the CH, PH, AP for the treatment of hypertensive patients at the background of atherosclerotic damage to the major arteries of the lower extremity with different VNS types prevailing, have been suggested and grounded.
