Dissertation work is devoted to depression of frequency of not incubation at women groups of high risk on a basis studying of level 25OHD3, clinical-functional, immunologic and microbiological features, and also development and deployment of advanced algorithm of diagnostic and treatment-and-prophylactic actions. New aspects of pathogenesis of not incubation of pregnancy at women with the burdened genesial anamnesis which consist available correlation interrelations between level 25OHD3 and immunity indicators at systemic and local levels are established: in blood the return interrelation between level 25OHD3 with a pro-inflammatory cytokine of IL-8 (Rs =-0,54; р <0,05) and a straight line - with TGF1 (Rs=0,70; р <0,05); in cells of the cervical channel the return interrelation between level 25OHD3 and pro-inflammatory cytokine of INF (Rs =-0,77; р <0,05). The direct interrelation between level 25OHD3 and probability of development of infectious process is shown during pregnancy (RR=1,2; 95% CI: 1,023-1,41, р <0,05). The infectious strain at pregnant women with deficiency 25OHD3 by 1,2 times exceeds at patients with failure 25OHD3 (<30 ng/ml) and by 2,8 times exceeds with an infectious strain of patients with normal level 25OHD3 (RR=2,78; 95% CI: 1,65-4,69, р <0,05). It is scientifically proved that development of not incubation in pregnant women with the lowered level 25OHD3 is bound to existence of the admixed bacterial and virus infection: at 41,7% of cases (?2=15,13; F=0,005; p <0,05). Thus, the bacterial and virus infection is characterized by existence of Mycoplasma genitalium, Ureaplasma urealiticum, Chlamydia trachomatis in cells of the cervical channel and DRA CMV, HSV 1 and 2 of type, EBV in blood and cells of the cervical channel, and also existence bacterial vaginose and Candida albicans. The algorithm of diagnostic and treatment-and-prophylactic actions for not incubation depression at women with the burdened genesial anamnesis with use holecalcyferol in dosage of 800-1000 IU is improved and introduced at a failure 25OHD3 (20-30 ng/ml) and 1500-2000 IU at deficiency (<20,0 ng/ml) since pregravidarne stage and throughout pregnancy under level 25OHD3 control. For control of efficiency of treatment-and-prophylactic actions by the most informative indicators at systemic level the contents in blood of pro-inflammatory cytokines of IL-8 and TGF1, and on local - in cells of the cervical channel level of a pro-inflammatory cytokine of INF is.
