Essay

The thesis made the theoretical generalization and new solution of the problem of studying the clinic, diagnostics, treatment and prevention of transient ischemic attacks (TIA) in the vertebral-basilar system (VBS) in patients with vertebrogenic pathology of the cervical spine (VPCS). A total of 130 patients with VPCS who underwent TIA with VBS aged 54.1 ± 1.2 years were examined. The incidence of TIA in VBS was predominant in women (70,8%). All patients showed changes in hemodynamics in the vertebral artery (VA), which was manifested by bilateral decrease or significant asymmetry of maximum systolic blood flow velocity at the level of segment V2 compared with segment V1. In all VA segments the left VA compression at head rotations on the background of a significant difference in the ratio of the diameter of the bone channel of the cervical spine to the diameter of the VA were determined. When the visual analyzer controls were switched off in the stabilometric test, the patients observed a violation of the stability of the main stand, as evidenced by the increase in oscillations of the pressure center (PC) in the frontal and sagittal planes, the CT velocity, the area of the statokinesiogram and the rate of its change. Differentiation treatment algorithms have been developed, the adequacy of which was confirmed by discriminant analysis.
