Essay

The dissertation is devoted to early diagnosis and treatment of acute appendicitis using a computer automated decision support system. The study was the first to study the diagnosis of appendicitis using a computer-aided diagnosis system using laparoscopic images, as well as the first study of the effectiveness of early laparoscopic diagnostics for further surgical care through laparoscopic surgery using an ultrasonic dissector. For the first time, it was found that the sensitivity and specificity of the diagnosis of appendicitis using a computer-aided diagnosis system was 89.3 % and 93.3 %, and the positive and negative prognostic indicator was 91.9 % and 89.4 %, which significantly exceeded the same parameters with standard laparoscopic diagnosis. The diagnostic efficiency of early forms of inflammation of the appendix with a reliable more than twofold decrease in the number of false-negative results in comparison with standard diagnostic laparoscopy was determined. The developed system of automated computer laparoscopic diagnosis of inflammatory diseases can be used during differential diagnosis and determination of indications for surgery for appendectomy. The possibility of transmitting digital images by telecommunication lines is the basis for the prospect of a positive use of the developed system in the provision of telemedicine care for surgical patients in rural areas.
