The dissertation presents a theoretical generalization and a new solution to the urgent problem of obstetrics and gynecology, in particular reproductive medicine - to increase the efficiency of reproductive function in infertile women with endometrioid ovarian cysts over 3 cm in diameter and reduced ovarian reserve. Based on a retrospective assessment of the impact of endometrioid cysts over 3 cm in diameter and their surgical treatment on the ovarian reserve in women with infertility who require in vitro fertilization, prospective study of clinical history, hormonal profile, echometric data of the pelvic organs, biochemical, ultrasound and histological parameters of ovarian reserve, proliferative activity in endometrioid cysts and in the surrounding ovarian tissue, embryological and clinical results with different algorithms in vitro fertilization cycles in women with endometrioid cysts of the ovaries more than 3 cm in diameter and with a reduced ovarian reserve, the optimal algorithm for the management of such patients is proposed, which includes the appointment of a preconception course of GnRH agonists, aspiration drainage of endometrioid cysts with sclerotherapy with ethanol and subsequent in vitro fertilization with controlled ovarian stimulation increased doses of gonadotropins, vitrification of the obtained embryos, transfer of vitrified / warmed embryos in a segmented cycle.
