The dissertation is devoted to increase of efficiency of diagnostics of structural and functional changes of right ventricle (RV) and influence of combined antihypertensive treatment on structural and functional indices of right heart chambers by a complex of echocardiographic methods in patients with hypertension and normal left ventricle ejection fraction. It is established particularities of right heart chamber structural remodeling in patients with hypertension with major cardiovascular risk factors. It is extended scientific understanding of the RV diastolic and systolic function in response to increased blood pressure. Independent predictors of diastolic and systolic RV dysfunction in this category of patients have been identified. Cardiovascular risk factors that affect the deterioration of RV diastolic function have been identified. It is proved that both diastolic and systolic parameters of longitudinal RV myocardial kinetics are closely dependent on remodeling and changes in the functional state of the interventricular septum. The use of effective combined antihypertensive treatment, which aims to suppress the renin-angiotensin-aldosterone system, has a positive effect not only on the left heart chambers, but leads to the reverse remodeling of the right heart chambers and improve RV diastolic function.
