Essay

The thesis presents a theoretical generalization and presents a new solution to the current scientific problem of modern obstetrics for optimization of diagnostics and management of conducting the pre-gravidar stage in women with isoimmune conflicts by improving diagnostic and treatment-and-prophylactic measures. The conducted analysis showed that at the first place the significance of perinatal risk factors for isosensitization by rhesus factor is occupied by miscarriages (χ2=7.58, p=0.000000), in the second place there is a threat of termination of pregnancy (χ2=23.33; p=0.000001), at the third place — the number of pregnancies more than three (χ2=17.06; p=0.000036). There is a moderate increase in blood lactate levels in the group of women with rhesus sensitization relative to the indicators of healthy women and are statistically significant (F=28.956; p=0.000). As a result of the study of blood bicarbonate levels revealed a slight decrease in its level in the group of women with rhesus sensitization (F=23.546; p=0.000). According to the ROS analysis, the quality of the statistical model of the use of intravenous immune globulins corresponds to a “very good” level — 0.843 (95% CI 0.689–0.941), the area under the curve in women in the comparison group was consistent poor model quality — 0.58 (95% CI 0.405–0.742). The efficacy of the use of intravenous immune globulins corresponds to the high quality of the studied statistical model and allows to recommend for carrying out pre-gravidar preparation of women with rhesus-isosensitization. Key words: isoimmune conflict, perinatal pathology, pregnancy, prevention.
