The study included the results of examination and treatment of 120 patients with severe acute pancreatitis. A comparative characterization of the dynamics of indicators that characterize the metabolic and inflammatory response, against the background of the use of different methods of nutritional support in patients with severe severe acute pancreatitis. It was found that the combined early enteral nutrition and parenteral nutrition allowed to achieve the target 100% caloric intake for 5 days intensive care in subgroups with a combination with parenteral nutrition mixtures in three-chamber bags (25.6 kkal/kg), compared with patients who underwent complete parenteral nutrition, and was accompanied by faster recovery of total blood protein levels on days 7 and 14 of intensive care. It was proved that complete parenteral nutrition mixtures in three-chamber bags, balanced by macronutrients, on the 5th day of intensive care provided calories by 68.0% of the target. The addition of w-3 fat acids in the complete parenteral nutrition of three-chamber bags contributed to a faster normalization of blood glucose, total serum protein and was accompanied by a faster normalization of blood lymphocytes (p = 0.042) and C-reactive protein (p = 0.052).
