Essay

In the dissertation the scientific justification of the efficacy of INRS for the adolescent with CP has been made, implemented and evaluated on the basis of the study of their clinical psychopathological and clinical psychological peculiarities and the level of social and psychological adaptation. The representation of the main clinical variants of psychopathological disorders of residual organic genesis in adolescents with CP (mild cognitive disorder and organic emotional labile disorder) has been established. In general, these patients were dominated by cerebrastenic symptoms and signs of cognitive impairment of varying severity. Peculiarities of formation of main cognitive and emotional personal disorders in adolescents with CP have been investigated. The main cognitive dysfunctions (memory impairment, voluntary attention, decrease of speed parameters of mental functions, low level and exhaustion of mental activity) depending on the clinical form of residual organic mental disorders on the background of CP has been established. However, the intellectual development of these adolescents is within the age norm. Typical features of their emotional sphere are the presence of various forms of character accentuations, as well as pronounced emotional lability, anxiety, sensitivity, rigidity. The role of cognitive, emotional and personal disorders in the formation of states of sociopsychological maladaptation of adolescents with CP is determined. A special algorithm for applying of INRS by V.I. Kozyavkin for correction of cognitive and emotionally unstable personality disorders in adolescents with CP has been scientifically substantiated and developed. A special algorithm for the use of INRS consists of a basic and additional set of rehabilitation measures. The main complex includes: biomechanical correction of the spine, joint mobilization, special massage system, apitherapy, reflexology, mobilizing gymnastics, communicative training with patients and their parents. The additional complex included: computer game therapy and robot therapy, art therapy, functional music therapy, rhythmic gymnastics, vibrotherapy, light therapy, mechanotherapy, gait correction on the treadmill, occupational therapy, orofacial massage. Approbation was performed and the effectiveness of the use of INRS for the rehabilitation of adolescents with CP with mental disorders of residual organic origin was evaluated. It is demonstrated that a significant improvement in mental status was recorded in 71.23% of all patients with CP who underwent rehabilitation with INRS, (including 61.3% of patients with mild cognitive impairment and 78.57% of patients with organic emotional labile disorder), improvement – in 21.92%, (respectively, 29.03% and 16.67%), changes were absent – in 6.85%, namely, positive dynamics was observed in 93.15% of patients. An increase in the level of intellectual productivity was found: the number of patients with average normative indicators increased (54.8% of patients compared to 43.8% before treatment), decreased – with low normative (31.5% and 45.2%). There was a clear improvement in the level of mental capacity, which was reflected in a significant increase in the amount of information processed and improvement of speed characteristics of thought processes. Positive changes were demonstrated in the increase of volumes of random access memory and long-term memory, improving the processes of concentration, distribution and switching of attention. It is demonstrated that in adolescents during rehabilitation with INRS significantly were reduced asthenic disorders, cognitive and emotional disorders, increased levels of intellectual productivity. INRS of prof. V.I. Kozyavkin is a modern rehabilitation system that can be used for effective correction of cognitive and emotionally unstable personality disorders in adolescents with CP.
