Essay

The study was based on the results of a comprehensive survey of 120 women whose men had paranoid schizophrenia (PS) and 50 women whose men did not have mental and behavioral disorders. The study confirmed the hypothesis of the presence of husbands with PS, signs of mental, family, psychosocial and personality-behavioral maladaptation of different content and severity, the constellation of which is a clinical picture of the holistic phenomenon of psychological maladaptation of women, differing depending on the disease. It can be realized in two clinical and psychological variants: subdmaladaptive, connected-dependent, peculiar to wives of men with duration of a mental illness to 8 years, and maladaptive disconnected-remote, characteristic of wives of men with disease duration from 8 to 15 years. In the course of research was proved that in the process of formation and dynamics of development and progression of psychological maladaptation the leading place belongs to stress-coping behavior, which is descriptor and predictor in same way. The descriptive role is played by the actual coping repertoire of the individual, the predictor – by the discontinuity of the coping resource base and the intensity of stressors arising from a man's with PS disorder, which causes the transformation of the initial deficit at the initial stage of maladaptation as a development of maladaptive process. A model of psychogenesis of psychological maladaptation in the wives of men with PS, according to which the primary biologically determined deficit of stress resistance provoked unjustifiably rapid depletion of communicative coping resources, which led to increased vulnerability to psychosocial stress factors. As a result, discordant interdependent relationships were formed, which led to the development of family maladaptation, which, in turn, created problems of psychosocial functioning, which, in turn, led to the formation of maladaptive mental states and further reduced the ability to stress, thus closing range of pathological interactions. Based on the created model of psychogenesis of psychological maladaptation in wives of men with PS, a program of measures for its medical and psychological correction was developed, the essence of which is to increase stress-protective resources and optimize stress-coping behavior as a core pathogenetic link in the development of maladaptation. use of psycho-educational, cognitive-behavioral, family and group personality-oriented influences, which are realized in eight successive stages. The implementation of this program has proven its effectiveness, so it can be recommended for widespread implementation in clinical practice.
