Essay

The thesis is devoted to increasing of treatment efficacy of chronic heart failure with reduced ejection fraction (CHFrEF) and comorbid iron deficiency (ID) as based on studying the peculiarities of the development of CHF in different hematological variants of iron metabolism disorder, clinical and laboratory evaluation of the efficacy of a continuous oral ferrotherapy and its combination with L-carnitine. It has been established that low hemoglobin content and iron transferrin violence are independent clinical and laboratory predictors of important course and decrease in quality of life (QOL) in patients with CHF from satellites of diabetes. Correction of concomitant latent ID and iron deficiency anemia (IDA) with iron sulfate for 6 months leads to normalization of hematological parameters, accompanied by a decrease in the functional class of CHF, improved exercise tolerance (ET), structural and functional state of the myocardium and an increase in QOL levels in the MLHFQ, physical and psychological components of health according to MOS SF36. The advantages of combined oral administration of iron sulfate with L-carnitine in patients with concomitant IDA in terms of exposure to ET and QOL.
