Essay

The dissertation is a clinical research and is devoted to increase of efficiency of sanatorium treatment of patients with osteoarthritis in combination with pathology of cardiovascular system by differentiated application of medical and rehabilitation complexes with inclusion of methods of peloidotherapy depending on degree of cardiovascular risk. The object of the study was: the condition of patients with osteoarthrosis of the knee joints in combination with pathology of the cardiovascular system.The subject of the study was: the effect of sanatorium-restorative treatment with differentiated use of peloid therapy on the clinical course, functional status, laboratory examination data and quality of life in patients with osteoarthritis of the knee joints in combination with pathology of the cardiovascular system. The work is based on the results of dynamic monitoring of 130 patients with osteoarthritis of the knee joints in combination with hypertension of I or II stage and 1˗2 degree, coronary heart disease, diffuse cardiosclerosis or stable angina pectoris I-II functional classes, 54,6±2,5 years old, of which 55 men (42%) and 75 (58%) women, who underwent sanatorium treatment in the clinical sanatorium «Arcadia» of the State Border Service of Ukraine. The results of the study showed that the use of developed differentiated peloidotherapy and therapeutic dosed Nordic walking on the background of the basic complex of sanatorium treatment improved the clinical condition of patients (reducing manifestations of pain in the knee joints, incidence and intensity of angina pectoris); had a positive effect on the daily profile of blood pressure and the presence of arrhythmias (extrasystoles), improved physical state and quality of life of patients with different degrees of cardiovascular risk. Patients of all groups received the basic complex of sanatorium treatment, which included: climatotherapy according to seasons, diet therapy (table No. 10 according to Pevsner), daily training in gymnastics (LH), magnetotherapy (MT) of the segmental zones of the heart, low-frequency alternating magnetic field No. 10; massage of the collarbone of the spine by braking technique, every other day, № 10. All patients received drug therapy according to the existing standards of treatment of cardiovascular disease in Ukraine. Modern and highly informative research methods were used to evaluate patients' condition and comparative evaluation of treatment results: general clinical (clinical examination, anthropometric data), anamnestic, instrumental (measurement of blood pressure, electrocardiography, daily monitoring of blood pressure and electrocardiogram), laboratory (lipid profile) blood, coagulogram, level of metabolites of nitric oxide, analysis of blood for uric acid, C-reactive peptide), functional (assessment of the level of physical workable Those definitions joint indices - Lekena, joint, pain, honiometrychnoho the definition of functional failure of joints), psychometric (assessment of quality of life questionnaire EuroQol and visual analogue pain scale) statistics. The studies were performed at baseline, immediately, and 6 months after the course of restorative treatment. According to long-term observation (6 months) high efficiency of the developed medical and rehabilitation complexes was revealed, which was manifested in the preservation of physical activity, improvement of the functional state of the joint apparatus and cardiovascular system, improvement of quality of life.
