Mokhnatyi S. Surgical treatment of severe mitral regurgitation in mitral valve prolapse.

The dissertation is devoted to definition of the factors influencing results of the mitral valve repair at the severe regurgitation of the mitral valve which is caused by a prolapse of mitral leaflets. For the purpose of the study, a retrospective analysis of medical histories of 146 patients who underwent mitral valve repair was performed. General clinical data, features of the main and concomitant pathology, anatomical and pathological features of mitral valves of patients, as well as features of surgical interventions on the mitral valve depending on the anatomical variant of prolapse of its leaflets are analyzed. The most important factors influencing the outcome of surgical treatment of patients with severe mitral regurgitation in mitral valve prolapse have been identified. These include the method of annular plication of the valve ring, the use of the method of resection of the posterior leaflet and the use of artificial chords. The influence of the anatomical variant of mitral valve prolapse on the postoperative outcome of surgical treatment of this pathology has also been proved. It is established that the height of coaptation of mitral valve leaflets is one of the main quantitative indicators of the quality of mitral valve repair. A prognostic model is proposed, which allows to determine the probability of valve repair surgery in the remote period for each patient.
