The dissertation is devoted to the topical issue of modern obstetrics, namely the improvement of childbirth, the condition of the mother, fetus and newborn by improving the diagnosis and pathogenetic justification for the discoordinated birth activity (DBA) correction. Based on the study 120 women in labor, scientific data on clinical and pathogenetic features and the nature of DBA, taking into account biochemical, endocrine, Doppler, cardiotocographic, morphological parameters. The role of phospholipids was studied for the first time and data on their importance in the development of DBA, the occurrence of perinatal complications were obtained. Scientific data on the comparative analysis of the main characteristics of the development of contractile activity of the uterus in DBA depending on different types of treatment. The use of phosphatidylcholine in the DBA correction, its effect on the course of labor, the condition of the fetus and newborn is theoretically substantiated. The peculiarities of the acid-base state of the fetal blood, the content of phosphatidylcholine and the condition of the newborn depending on the method of treatment of DBA were established. The study analyzed the morphological changes occurring in the placenta when using different types of DBA correction, and determined different degrees of damage to the placenta and its vascular sector. The algorithm of complex medical and diagnostic tactics for the DBA correction has been expanded, taking into account the content of biologically active substances and phospholipid fractions in the blood of the woman in labor, the state of the fetoplacental complex
