Essay

The dissertation contains the results of medical and surgical treatment of 144 consecutive patients with dilated cardiomyopathy who had severe secondary mitral regurgitation from 2007 to 2017 at Amosov National Institute of Cardiovascular Surgery of the National Academy of Medical Sciences of Ukraine. The paper presents an analysis of the impact of the initial clinical and laboratory data and other factors on the course of dilated cardiomyopathy. The immediate and long-term results of medical treatment (including cardiac resynchronization therapy according to indications) were studied in 84 patients from the comparison group, which showed low efficacy confirmed by the mortality rate within a year in 16.0% of patients, and in 48% and 66.7%. cases for three and five years, respectively. Surgical treatment of atrioventricular valve insufficiency by restrictive annuloplasty was performed in 60 patients with dilated cardiomyopathy. It was found that with an end-diastolic index of more than 197 ml/m2, it is recommended to accompany the restrictive annuloplasty with the papillary muscle approximation and formation of the supporting frame of the left ventricle.
