The dissertation is devoted to the study of the peculiarities of the state of the female reproductive system in adolescent girls with a pathological course of puberty, characterized by the presence of dysmenorrhea syndrome. We have investigated the endocrine state of the patient's body depending on the nature of metabolic disorders, the severity of the pain syndrome, the state of metabolism of eicosanoids, taking into account the extragenital background. Multifactorial correlations were established between pro-inflammatory cytokines, prostaglandins and indicators of endothelial dysfunction, their relationship and role in the pathogenesis of primary dysmenorrhea of varying severity. The data obtained allowed us to form high-risk groups with a pathological course of puberty, improve early diagnosis and develop pathogenetic correction of the most severe menstrual dysfunctions associated with the peculiarities of the functioning of the female reproductive system developing against the background of metabolic changes of the surveyed contingent. Complex analysis of the clinical course of dysmenorrhea, taking into account the nature of physical, sexual development, hormonal levels, characteristics of metabolic disorders and the nature of the formation of menstrual function, depending on the severity of pain syndrome and the exchange of eicosanoids, allowed us to develop effective therapeutic and prophylactic measures for these disorders in adolescence, which should contribute to preservation of the reproductive potential of this contingent.
