The dissertation work is devoted to improving the diagnosis of arterial hypertension in middle-aged and elderly patients based on assessing the influence of hemodynamic and genetic factors on changes in the structural and functional state of the heart and blood vessels and optimizing the control of the effectiveness of antihypertensive therapy based on long-term follow-up data. The diagnostic tactics of examination of patients with hypertension was improved on the basis of the data obtained as a result of the study. There were established the most significant parameters which affects the development of pathological changes in the structural and functional state of the heart in patients with hypertension, depending on age. It has been proved the necessity of determining the synchronicity of the circadian rhythms of systolic blood pressure and diastolic blood pressure, calculation of the BP uniformity index, the index of the prevalence of hypertension in hypertensive patients in the elderly and in middle age during ambulatory blood pressure monitoring to identify violations of the chronobiological structure of BP. It was established the importance of determining the genotype of the renin-angiotensin system genes in middle-aged and elderly hypertensive patients for early detection of the risk of left ventricular diastolic dysfunction.
