The dissertation is devoted to the topical issue of endocrinology, which is to deepen the modern scientific understanding of the associative pattern of hormonal, hemodynamic and inflammatory disorders in the genesis of hypertension on the background of obesity in middle-aged Ukrainian population to improve the prevention of this combined pathology in this category. Priority data on the characteristics of fluid sectors of the body in middle-aged people with obesity and hypertension: it is shown that in this population there is a complex of direct links between the absolute accumulation of total water, total, intracellular, interstitial fluid and active cell mass growth, and also levels of systolic and diastolic blood pressure, the volume of circulating blood in the body. But this connection is rather weak. It is shown that the additional accumulation of water and fluid in the body is not a dominant factor in the development of hypertension in middle-aged people. Relationships between renin and aldosteroneemia levels and the degree of insulin and leptinemia, the severity of low-intensity chronic inflammation, and the degree of peripheral tissue sensitivity to insulin in middle-aged people in the Ukrainian population have been reported to be highly probable, direct, and moderate. It has been shown that the combination of obesity with hypertension is a factor that causes an additional increase in total peripheral vascular resistance. Additional evidence for the leading role of visceral obesity in the development of hypertension has been obtained, but data have been obtained that suggest a possible effect of gluteofemoral fat depot on the development of diastolic hypertension. It was determined that the level of serum clusterin can be considered a marker of the risk of hypertension in the Ukrainian population of obesity. It is established that the determining factor for the development of hypertension in the middle-aged Ukrainian population with obesity is the severity of chronic inflammation of low intensity on the background of the same degree of insulin resistance, leptin and reninemia, total peripheral vascular resistance - which distinguishes (according to discriminant analysis). pattern of patients with associated pathology of obesity and hypertension from normotensive individuals with obesity. The optimal approach to a comprehensive examination of middle-aged people regarding the possibility of early detection and prediction of hypertension in the presence of obesity, namely: cumulative determination of circulating clusterin, renin, aldosterone, leptin, insulin, HOMA-IR, relative fat mass, systolic blood pressure, systolic blood pressure circulating blood, shock index, total peripheral vascular resistance. Using the methods of discriminant analysis, methods for predicting the development of hypertension in the middle-aged population with obesity, which are characterized by sufficient sensitivity, specificity and accuracy.
