Fetisov S. Improvement of surgical treatment of patients with progressive chronic lower limb ischemia

This study is based on the analysis of case histories, the results of clinical laboratory and instrumental examination and treatment of 203 patients with obliterating atherosclerosis of the vessels of the lower extremities. The patients were divided into two groups: the main group and the comparison group. The main group consisted of 102 patients in whom the tactics of examination and treatment developed by us (both conservative and operative) were applied. The comparison group consisted of 101 patients in whom standard methods of diagnosis and treatment of lower limb ischemia were used. Improved tactics for the treatment of chronic ischemia in patients with atherosclerosis, based on the expansion of the orifice of the outflow tract and the preservation of all collaterals, control of the blood lipid spectrum, made it possible to reduce the number of shunt occlusions from 12.3% to 2.1%, amputations from 10.7% to 5.4%, and mortality from these complications from 13.8 to 3.2%.
