Object - type 2 diabetes mellitus, late microangiopathic complications on the background of type 2 diabetes mellitus; The aim is to experimentally elucidate the relationship between changes in the characteristics of the hemostasis system, antioxidant protection and endothelial dysfunction in late diabetic complications, as well as pathogenetically substantiate the effectiveness of bioflavonoids in combination with L-arginine; methods - pathophysiological, hematological, biochemical and statistical; results - supplemented scientific data on the pathogenetic mechanisms of diabetic vascular complications; For the first time, the prognostic significance of assessing the state of vascular and platelet and coagulation units of the hemostasis system was established, taking into account the severity of oxidative stress and changes in the antioxidant defense system and lipid peroxidation in late complications of type 2 diabetes. It has been shown that in diabetic retinopathy and diabetic nephropathy there is an increase in intravascular platelet activation and a decrease in antiplatelet activity of the vascular wall, which confirms the opinion that normalization of glycemia does not normalize platelet function; the role of systemic disorders of coagulation hemostasis in the development of diabetic vascular complications is shown; developed a comprehensive methodological approach to the prevention and correction of microangiopathic complications on the background of type 2 diabetes by using bioflavonoids and L-arginine.
