106 patients with ischemic heart disease were examined with the aim of enhancing of efficacy of treatment of the patients with unstable angina basing on studying of pathogenic mechanisms and prognostic value of silent myocardial ischemia and comparative evaluation of different regimens of therapy. It was estimated that destabilization of coronary heart disease was accompanied by decreasing of frequensy, mean and total duration of silent myocardial ischemia. The role of lipid exchange and bloode coagulation system disorders in development of episodes of silent myocardial ischemia was shown. The criteria of unfavorable short-term outcome in patients with unstable angina were estimated. The different regimens of antianginal therapy of patients with unstable angina and episodes of silent myocardial ischemia including beta-blocker atenolol and calcium antagonist diltiazem were worked out. The unfavourable influences of persistence of episodes of silent myocardial ischemia during antianginal treatment and li pid exchange disorders on long term outcome in 8 years were shown.
