Vereshchako R. Reconstructive surgery of lung cancer.

Problems of surgical treatment of lung cancer, in particular different tupes of resection and reconstruction of the carina and major bronchi, are discussed in the thesis. Diagnostic criteria and indications for diffirent types of reconstructive procedures have been elloborated on the basis of clinical analysis of patients with lung cancer. New methods of resection and reconstruction of the carina and major bronchi have been proposed; the details and surgical technique of establishing tracheobibronchial,tracheobronchial, polybronchial and bronchobronchial anastomoses have been improved. Changes of bronchoscopic view in postoperative period have been investigated. System of prophylaxis and treatment of postoperative complications has been ellaborated. Good direct treatment results have been obtained in 93 (96,9%) patients. Postoperative mortality rate made up 3,1%. 5-year-survival rate after lob(bilob-)ectomy with bronchial resection was 56,3%, but after isolated carinal resection or carinal resection co mbined with lob-, bilob-, or pneumonectomy-47%.
