Kozlov S. Evaluation of risk factors of the surgical tactic in the patients with bleeding pyloroduodenal ulcers

Dissertation based on results of surgical treatment of 573 patients. Risc factors of surgical treatment were worked out by stepwise discriminant analysis and defined as follows: age above 60 years, resection methods, severe grade of bleeding, urgent operation on the height of early ulcer bleeding recedive. Offered prognostic system permits to stratify patients into three groups of low, medium and high risc of bleeding recurrence according to calculated risc value. Terms of operation were choosen depending upon risc group. Special algorithm of surgical tactic and new surgical procedure for closely located to Vateri duct bleeding ulcers proposed. Postoperative complications under offered surgical tactic protocol decreased from 15,% to 7,1%, lethality from 4,5 to 1,5%.
