The thesis is devoted to the problems of surgical treatment of perforated gastric and duodenal ulcer. The character of inflammatory-diestructive changes of parietal and vesceral peritoneum at various terms since the perforation moment was determined. The influence of endogenous intoxication severity on the frequency of postoperative complications and lethality was established. The quantitative and qualitative analysis of exudate microflora of pertoneal cavity was carried out. The frequency and severity of hemodynamics disorders were investigated. On the basis of obtained results the means of surgical tactics improvement for the treatment of patients with perforated ulcer were proposed. The possibility of radical operations performing at the terms up to 10 hours since the moment of perforation was proved. The necessity of obligatory removal of ulcer substrate was substantiated
