Essay

We have established the succession of mechanisms of ischemic damage of the local renin-angiotensin system of the heart: ischemia - an increase of the intracardial content of angiotensin II - a decrease of tissue fermentative fibrinolytic activity of the heart - activation of the generation systems of oxygen radicals - activation of lipoperoxidation and peroxide modification of the myocardial proteins - disturbances of the electrolyte balance of cardiomyocytes. A complex use of inhibitors of the angiotensin-converting enzyme, AT1A-receptor blockers of the lozartan type and antioxidants to prevent ischemic-reperfusing lesions of the myocardium has been pathogenetically substantiated.
