Essay

The results of the clinical investigation of cytokines (interleukins 1?, 6, 8, 10, tumor necrosis factor ?, interleukin 1 receptor antagonist), trypsinogen activating peptide, and myeloperoxidase blood concentration in patients with acute pancreatitis of the different degree of severity are represented. Novel concept of pathogenesis of acute pancreatitis permitted to develop the principles of the rationale conventional therapy. For the first time was established that applying in the complex management of acute pancreatitis remedies with the anticytokines properties decreased the levels of circulating cytokines, inhibited their systemic effects and decreased the severity of multiple organ failure. On the basis of the complex evaluation of clinical signs, levels of mediators of inflammatory response the indices and terms of the surgical intervention in patients with necrotizing pancreatitis.
