Essay

The author carried out a complex examination of 6091 and 883 victims with polytrauma in the comparative groups. The traumatic illness is determined to assume the most widespread level of complications and lethal outcome under the influence of risk factors. The author determined that under the influence of risk factors the most of adaptation mechanisms exhausted faster and the traumatic illness took more serious course, the acute period of the traumatic illness reduced to 2 days and the stage of traumatic emaciation lasted from 5 to 16 days. The author has shown that the victims under the influence of risk factors have such peculiarities of clinical description of the traumatic illness as the considerable severity of damage, the traumatic shock; the development of mutual burden syndrome; a great number of complications and resistance to medical measures. Proposed by the author anatomy-functional index of determination of heaviness allowed to estimate severity and forecast the result of polytraumas. Proposed conception of differentiative surgical tactics allowed to reduce the frequency of the appearance of complications from 76.06 per cent to 60.38 per cent, common lethal outcome - from 56.20 per cent to 44.03 per cent.
