. Object of investigation: 234 patients after myocardial infarction The aim of investigation: to work out the conception of prognose, profilactic and treatment of heart failure and sudden cardiac death on the basis of investigating of structure-functional changes of left ventricular, inflammation and neurohumoral breaches in patients after myocardial infarction. Methods of investigation: clinical examination of patients, electrocardiogram, echocardiogram, X-ray of chest, Cholter minitoring of ECG, signal-averaged ECG, immunofermental methods of cytokines, brain natriuretic peptide, aldosterone; immune - von Villebrand factor, C-reactive proteine, biochemical methods - fibrinogen, noradrenaline. Theoretical and practical results: it has been picked out predictors of afterinfarction remodelling that helped to search patients with the threat of appearance of heart failure, sudden cardiac death. On the basis of multifactor analysis the model of estimation of vulnerable patient was done. It has been provedscientifically positive influence of carvedilol and ACE-inhibitors on reverse development of left ventricular remodeling and its prevent, repeated ishemic episodes frequency, that prove to heart failure, arrhythmias decrease. The extent of introduction: Investigation's results were introduced to the work of therapeutical, cardiological and reanimatological departmens of clinics of Kharkiv, Kyyv, Sumy, Poltava, Institute of therapy AMS of Ukraine; Kharkiv State Medical University, Kharkiv Medical Academy of Afterdoploma Education Sphere of application: medicine, cardiology.
