Essay

The pathophysiological changes in patients with temporary or long-term violation of intestinal tract continuity during surgical operation and spontaneous appearance of intestinal wall defects in the form of fistulae are explored. The pathophysiological mechanisms of interaction between devices for intestinal fistulae obturation and intestinal wall in the area of fistulae defects are studied. Improved constructions for temporary reconstruction of intestine tract continuity in case of spontaneous fistulae appearance as well as in case of completion of urgent small intestine resection by terminal stomas forming are developed. Algorithm of completion of urgent small intestine resection depending on patient's condition and operation situation was developed. Improved tactics of treatment of patients with intestinal fistulae depending on the number and quality of losses through fistulae defects is grounded. Application of suggested approach made it possible to improve the results of surgical treatment of patients with spontaneous and artifitial bowels defects. So common lethality decreased from 19.48% to 9.23%, postoperational lethality dropped from 10.5% to 2.9% and the number of postoperational complications diminished from 20.8% to 9.4%.
