Essay

Thesis is devoted to the problem of diagnosis and complex treatment of several pyo-septic processes of abdominal cavity and/or retroperitoneal space- abdominal sepsis. Performed experimental and clinical examinations revealed maximal disorders of macrophagal-endotelial correlations on the level of small intestine wall up to destruction of cells with possible activation of cytokine cascade, gross disorders of oxygen metabolism and intestinal biocenosis what allows to observe the arrising enteral insufficiency as the leading pathogenetic factor of abdominal sepsis and multiorgan insufficiency. The frequency and main causes of arising of abdominal sepsis were studied and were developed original methods and devices for intubation of intestine, methods of endotoxicosis and enteral insufficiency treatment. There were proposed working plans for treatment of endotoxicosis and enteral insufficiency. Algorithm of treatment of patients with abdominal sepsis was developed. The developed medical-tactical programallowed considerably to improve results of surgical treatment of patients with abdominal sepsis what manifested in decrease of pyo-septic complications from 39,7 to 27,4 % and lethality from 35,7% to 21,5%.
