Chervonopіska O. Remodeling of the left ventricle and changes of the inheart hemodynamic during the phase of heart failure in patients with ischemic and inflammatory affections of the myocardium.

The dissertation is dedicated to one of the currently actual problems of the cardiology - the perfection of the early diagnostic of the heart failure (HF) on the basis of using of the complex echocardiography research of the heart. The broad possibilities of the impulse-wave Doppler in estimation of the effectiveness of treating interference during early and independent periods have been demonstrated there. On the basis of this method there has been studied course of the structure-functional rebuilding of the left chambers of the heart in patients with stable forms of the IHD in consequence of the revascularization of the left ventricle (LV) after the coronary-artery stenting. The role of the phenomena of the mitral regurgitation in development of the dilatation of the LV chamber in case of the chronicle ischemic affection has been determined. There have been given characteristics of the diastolic filling of the LV according to the abnormal relaxation type and of the restrictive type of the diastolic filling, i.e. of the syndromes, which show the periods of the compensation and/or decompensation of the remodeling in case of HF. It was proved, that the changes of diastolic function of the LV (parameters of the diastolic mitral flow, structure-functional state of the left atrium, end diastolic volume and geometric indices of the LV during the diastolic phase) do not depend on the influence of the etiological factor and show common regularities of remodeling. These changes are early diagnostic criterions of HF.
