Doctorate thesis is devoted to the medical problem of current importance – anesthetic and reanimation management of extensive peritonitis patients. Experimentally researched are disorders of structural water state of tissues in peritonitis, described is their role in pathogenesis and studied are restructuring resources. Determined is the influence of certain agents, used for anesthetic and reanimation management, on translocation of intestinal flora in the process of experimental peritonitis development. An in-depth research of biological reactions in peritonitis was carried out in the clinic. Defined are 8 phases of these reactions’ processing, which served as a base for development of recovery concept of biological integrity of human organism in peritonitis. In accordance with peritonitis stage, severity of septic process and phase of disturbance of biological integrity, developed are 4 programs for preoperative preparation, anesthetic management and postoperative intensive care. Application of proposed methods excluded mortality in reactive phase of peritonitis, ensured decrease of lethality down to 8,9% in toxic stage and down to 31,6% in terminal stage of extensive purulent peritonitis.
